Relationship to Ability of Discourse Comprehension and IADL in Patients with Amnestic MCI and with Mild Dementia of Alzheimer¡¯s Type

Abstract
Objectives: The purpose of this study was to investigate the relationship between instrumental activities of daily living (IADL) and ability of discourse comprehension in patients with amnestic mild cognitive impairment (aMCI) and with mild dementia of Alzheimer¡¯s type (DAT).

Methods: Thirty-one patients with aMCI and 30 patients with mild DAT participated in this study. IADL was assessed using the Korean version of the Lawton IADL index, and discourse comprehension was assessed using a recall task that involved answering simple questions and listening to the discourse. The discourse comprehension task questions consisted of literal information (LI), text-connecting inference (TC), and gap-filling inference (GF).

Results: First, the age and education level were not correlated with the IADL performance of the aMCI and the mild DAT groups. However, the higher the Korean version of Mini-Mental State Examination scores of the mild DAT patients, the better the performance of the IADL. Second, the IADL score of aMCI group showed a significant correlation with TC and total score of the discourse comprehension task. Third, the IADL scores of the mild DAT group showed a significant correlation with the LI, TC, and total scores of the discourse comprehension tasks.

Conclusion: Both aMCI and mild DAT patients were correlated with the performances of IADL and discourse comprehension. In particular, impairments of memory and executive function are related to the restriction of the IADL.
